New student-athletes (freshman, transfers, new team members)
All student-athletes, including first year participants and transfers are required to obtain a physical exam and immunization from their primary physician.  The physical should take place in the 6 months prior to the beginning of classes.  Immunization information can be found at www.njit.edu/healthservices. 

• General Physical Exam to be completed by primary physican.
• A signed copy of the Insurance Information Letter 
• Complete Intercollegiate Insurance Form
• Complete Medical Health History Questionnaire 
• Complete Sickle Cell Form
